K-12 Voluntary Student Accident - Eligibility

	Enter the first few letters of the school system name, choose the school state and click the SEARCH button.If your student's school is not listed, please call us at 1-855-742-3135.

	If you are enrolling more than one student, your first school system selected will be the default for all students. You will be given the opportunity to edit this selection by student later in the enrollment process.

	Note: K-12 Voluntary Student Accident coverage is not available in the following states: Arkansas, Maryland, New Hampshire, New York, Washington.


	You must select from one of the two options below before selecting your school:

The following instructions describe how to set JavaScript options for:

Mozilla 2.X & Above

Open Mozilla.
Select Options from the Tools menu.
Make sure Enable Javascript is checked.
Click OK.
Click Reload.

Internet Explorer 6.X & Above

Open Internet Explorer.
Select Internet Options from the Tools menu.
In Internet Options dialog box select the Security tab.
Click Custom level button at bottom. The Security Settings dialog box will pop up.
Under Scripting category enable Active Scripting.
Click OK twice to close out.
Hit Refresh.
